- Copy of valid Biodata page of International Passport
- Recent passport-sized photographs with a white background
- 3-Months stamped bank statement with reference letter from the bank - all pages should be stamped

- Completed and signed Application Form (B1-84 FORM) using black ink only.
- A valid international passport with a minimum of 6 months remaining validity and at least 3 consecutive blank pages
- Two recent passport-sized photographs with a white background
- Copy of Biodata page of International Passport
- 3-Months stamped bank statement with reference letter from the bank - all pages should be stamped
